Access To Patient Information
All patient information is considered confidential and we comply with the GDPR. All employees have access to this information and have signed a confidentiality agreement. Information may be shared with other NHS organisations in the interest of patient care. Confidential and identifiable information will not be disclosed without written consent, unless:
- It is a life or death situation
- It is in the public interest to do so
- There is a legal obligation to do so
If you require access to your medical records, please make a request in writing. Please ask at reception for a leaflet explaining GDPR in more detail
Categories of Data
Typically, Clinicians and Pharmacists will require access to patient information including NHS numbers and medication lists.
Complaints & Suggestions
Our Practice staff are here to help you, however if you feel you have been treated unfairly please speak to the Practice Manager, Dawn Threlfall who will be happy to help you. We welcome any suggestions you may have to improve our services.
Alternatively you can ask at reception for details of how to make a complaint and our complaints procedure.The information available explains fully our complaints process and the timescales on dealing with your complaint. We would however ask that in the first instance you request a meeting with the Practice Manager in order that we might be able to resolve any issues or concerns you may have.

GP Earning
All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.
The average pay for GP’s working in Dr Bisarya, Sandy Lane in the last financial year was £45,803 before tax and National Insurance. This is for 1 full time GP and 2 part time GPs who worked in the practice for more than six months.”
Patient Rights & Responsibilities
You have right to expect a high standard of medical care and we will try at all times to provide the best care possible. It is your responsibility to attend for appointments made and follow medical advice given.
Very occasionally patient/practice relationships break down. In this situation, the patient may choose to leave the Practice. The Practice also has the right to remove a patient. A specific reason would normally be given to the patient.

Important information for patient

- We share medical records with those who are involved in providing you with care and treatment.

Privacy Notice-Medicines Optimisation – Purpose and Legal Basis for Processing
West Lancashire CCG has a duty to secure continuous improvements in the quality of services provided to individuals for, or in connection with the prevention, diagnosis or treatment of illness. Taking this into account, The Medicines Management Team supports the CCG with commissioning services that make best use of available medicines. Your personal data will be used to fulfil this duty in respect of promoting cost-effective use of medicines as well as implementing projects or actions to optimise the use of medicines to improve outcomes, enhance patient safety and improve capacity within the local health economy.
Recipients of Personal Data
Personal data is shared between the CCG and local healthcare providers including GP practices. They do this to faciliatate the implementation of recommendations made by the Medicines Management Team.
Source of Data
Data used to fulfill the above duties is recieved directly from the primary and secondary healthcare providers for which the CCG has responsibility for.
Zero Tolerance
- The practice susbscribes to the NHS Policy of Zero Tolerance
- Any form of verbal, physical or written abuse or aggression is taken very seriously and may lead to removal from our practice list
- This includes any derogatory comments on social media/networking or internet sites
